Enrollment

Qualifying for enrollment requires the following:
• Medicaid funding including Tefra, ARKids A, Medicaid and/or SSI
• Qualifying scores on evaluations according to Medicaid guidelines in two of the following areas: Physical Therapy, Occupational Therapy, Speech-Language Pathology, IQ or cognition
• A prescription for services from your child’s primary care physician
